|3-302.11(A)(1) ||Corrected During Inspection Critical Repeat Raw animal food holding in a manner that may cause cross contamination of ready-to-eat food as follows: marinated raw chicken shelved over iceberg lettuce.|
Food shall be protected from cross contamination by separating raw animal foods like poultry, beef, pork, seafood, and eggs during holding from ALL other foods such as cooked foods, ready-to-eat foods, washed produce, etc.
|3-302.11(A)(4) ||Corrected During Inspection Critical Repeat Unwrapped or uncovered food in the following location where the food is subject to contamination: iceberg lettuce.|
Foods shall be protected from contamination by being stored in packages, covered containers or wrappings
|3-501.16(A)(1) || Critical Repeat The following food item(s) were found hot holding at improper temperatures using a calibrated food temperature measuring device: mini vegetable samosas observed at 120F in display case.|
Potentially hazardous foods shall be hot held at a temperature of 135°F or above to prevent potential bacterial growth.

6.501.111C - Controlling Pests by Methods:
Effective methods are not being used to control pests. Crawling insects observed in two locations.
Corrective Actions: Insects and other pests are capable of transmitting disease to people by contaminating food and food-contact surfaces. If pests are found on the premises methods shall be used to control their presence.
3-602.11(B)(1)-(7) - Food Labels/Common Name/Ingredients/Manufacturer
Observation: Insufficient information is provided on the label of the following food item(s) packaged in the food establishment: rice pudding, mango lasse
Correction: Food packaged in a food establishment for customer self service shall include labels that contain the following information: 1. the common name of the food, 2. the list of major ingredients including artificial colors or preservatives, 3. the total net weight or quantity of the food, 4. the name and place of business of the food manufacturer, packer, or distributor, 5. the name of the food source for each major food allergen contained in the food,6. nutrition labeling
3-305.11(A)(3) - Food Storage / Preventing Contamination
Observation: Food stored on the floor and/or food stored less than 6 inches off the floor.
Correction: Food shall be protected from contamination by storing the food at least 6 inches off the floor on approved shelving units

Observation: A la carte containers were observed stored unprotected
Correction: Single-service items shall be kept in the original protective package or stored by using other means that afford protection from contamination until used.
4-501.11(A) - Equipment / Good Repair / Operation
Observation (REPEAT): The grill True RI Unit is not operating properly and is unable to maintain cold food at or below 41°F
Correction: Adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly.
5-501.113(B) - Refuse / Cover Receptacles / Outdoors / Tight-Fitting Lids
Observation: Outside refuse container uncovered
Correction: Receptacles and waste handling units for refuse, recyclables, and returnables shall be kept covered with fitted lids
